Dear AQUILA Internet Services,
Focal Communications has provided you with a series of telephone numbers in various rate centers throughout the Chicago MSA. Each telephone number has a designated local calling
area as defined in our Focal Finder. Please be certain you are using the newest version of the Focal Finder.
Ameritech will bill calls made from telephone numbers we have defined as local to the area, to residential AQUILA customers as flat rate calls in the least expensive rate band.
If the call is billed at a higher rate, a toll charge, Focal Communications agrees to provide credit to the user for the entire cost of the call, including tax.
Requests for credit must be accompanied by the proper documentation. The documentation required is as follows:
- A copy of the user's local telephone bill, including full usage detail for the calls in question. (The local phone company can provide usage detail upon request from the user. Users
may receive call detail from the local phone company up to twice per year at no charge.
- Calls requiring credit should be highlighted or noted for easy identification.
- Contact name and telephone number and mailing address should be provided if other than listed on the local telephone bill.
Fully documented requests for credit should be mailed to:
Individual credit requests will be reviewed and processed within five business days of receipt.
Credit checks will be mailed to the address on the local telephone bill unless otherwise specified.
